IMPORTANT REQUIREMENT ISSUED FOR ALL STUDENT-ATHLETES PARTICIPATING IN THE 2023-2024 SEASON:

All student-athletes must get their physicals done off-campus before preseason training to limit numbers in the Health Services offices and overall exposure on campus.

Mission Statement         
The Our Lady of the Lake University Athletic Training Department exists to provide health care services to all student-athletes who participate in OLLU Athletics. The OLLU Athletic Training Department strives to prevent injuries, provide prompt treatment of injuries when they occur, and rehabilitate athletes whose injuries require restorative care. It will be the goal of the Athletic Training Department to administer its responsibilities with both professional skills, compassionate care, and provide the best possible medical resources necessary to allow our student-athletes to return to competition as soon and as safe as possible.  

 

For Student-Athletes trying out for Saints Teams
Many student-athletes are scheduled to be on campus throughout the year to try out for one of our OLLU's intercollegiate teams. Before actually trying out for the team, the student-athlete 18 years old and older must fill out the form linked below. Parents must fill out this form for those student-athletes younger than 18 and submit it to the coach before the student-athlete actually trying out. Click here for the required form.

 

Parents and Student-Athletes:
All of the information on this page is required for participation in Our Lady of the Lake University Athletics. We are asking for the forms to be completed and returned as soon as possible. These forms must be filled out before any OLLU athletics participation. These forms are to be renewed annually. In addition to these forms, we are asking for a copy of your current insurance card (both sides), copies of dental and/or vision cards, and immunization records (for new students).

All athletes must get a pre-participation physical completed and fill out the athletic training room required forms before they are eligible to participate in intercollegiate athletics. The required forms can be obtained online and then printed out for completion and mailing.

As an Our Lady of the Lake University athlete, you are required to complete and submit a medical history questionnaire, insurance information, waiver, and consent and release forms each year. Completion of all paperwork and the physical exam is required before you begin any athletic participation.

If you require a medical clearance for a health condition (e.g., heart condition) or injury (e.g., recent reconstructed ACL) from another physician, please arrange to obtain a signed release and send in your required forms below. If you have questions about if you should obtain a release, please contact the athletic training staff.

OUR LADY OF THE LAKE UNIVERSITY LIGHTNING POLICY AND PROTOCOL

General Statement: All individuals should have the right to leave a site or activity or refuse travel, without fear of repercussion or penalty, to stay safely home or seek safety if they feel that they would be in danger from inclement weather and/or lightning activity.

CONCUSSION PROTOCOL

The Concussion Protocol has been developed to help the Our Lady of the Lake University Sports Medicine department treat suspected concussed student-athletes efficiently and objectively. By following this protocol, the sports medicine department will be able to provide efficient treatment to keep the student-athlete's health primary and safe while also assisting the student-athlete in returning to competition as quickly as possible following a concussive episode.

All students must read the Student-Athlete Handbook. After reading the document, each student must sign the form below and turn it in to the Athletic Trainer.

ATHLETIC TRAINING REQUIRED FORMS

The athletic training department will be using the electronic medical record-keeping software, Healthy Roster.  The incoming athletes will need to download the Healthy Roster application to their cellphone and then email the athletic trainer (Abby Garcia, agarcia@ollusa.edu) with the following information:

  • Name
  • Date of birth
  • Cell number
  • Email address
  • Sport
  • Country (If International Student)

Athletes will be emailed an access code from Healthy Roster once a profile has been created. They will be able to upload a copy of their physical and insurance card to their profile.
